Cost is $25.00 Physician is Dr. Chow

There are three forms required with Parent signature.  Please have students see Mr. Riggs in PE for details.

All Students participation in 7th and 8th grade sports must have a physical on file.
The physical must be done within the school year (July to June)

6th grade students that participate in the spring for track must also have a current physical on file.

If you choose to use your own doctor you may pick up the required forms from the front office.
